DIF is a specialised test performed on a fresh skin biopsy where fluorescent antibodies are applied to detect immunoglobulin and complement deposits in the skin layers. It is essential for diagnosing autoimmune blistering diseases such as pemphigus vulgaris, bullous pemphigoid, and the lupus band test. The biopsy must be sent in Michel's transport medium, not formalin.
Both are components of the desmosome — the structure that binds skin cells together. In Pemphigus Vulgaris, anti-DSG3 antibodies target the deeper epidermis causing mouth ulcers and flaccid blisters. In Pemphigus Foliaceus, anti-DSG1 antibodies attack the superficial epidermis causing a scaly, exfoliative rash without mucosal involvement. This distinction directly guides treatment intensity.
We perform a KOH preparation from nail clippings to rapidly visualise fungal elements under microscopy. A fungal culture confirms the species (Trichophyton, Candida, etc.) and determines antifungal sensitivity — critical because different species respond to different treatments, and some nail dystrophies are not fungal at all.
No single blood test diagnoses urticaria, but investigation is essential to find treatable causes. A standard urticaria workup includes CBC with differential, ESR, CRP, thyroid antibodies (Anti-TPO, Anti-TG), ANA, IgE, Vitamin D, and a comprehensive metabolic panel. H. pylori testing is also increasingly recognised as a reversible trigger.
