The FRAT Test Kit (Folate Receptor Autoantibody Test) is a specialised blood test that detects autoantibodies against the Folate Receptor Alpha (FRα) protein — responsible for transporting Vitamin B9 (folate) across the blood-brain barrier.
When the immune system mistakenly produces these autoantibodies, folate cannot reach the brain in adequate amounts — even when blood folate levels appear completely normal. This condition is called Cerebral Folate Deficiency (CFD).
Research published in peer-reviewed journals indicates that up to 70–75% of children with ASD may have detectable Folate Receptor Autoantibodies (FRAbs).
Early detection through FRAT testing — supervised by Dr. Alok Gupta (MD Pathology, Ex-PGIMER, 12+ years) — can open the door to targeted folinic acid (Leucovorin) therapy, potentially improving neurological outcomes significantly.
The FRAT Test is clinically relevant for the following individuals. If your child's neurologist is considering Leucovorin therapy, this test is an essential first step.
Getting the FRAT Test used to mean travelling to a specialised diagnostic centre. Altus Lab has changed that — we bring the test to your doorstep, wherever you are in India.
Contact Altus Lab via phone, WhatsApp, or the booking form. Share your city, address, and preferred time slot. Call +91 6284 214 977 — same-day slots often available.
Our trained phlebotomist visits your home at the scheduled time and draws a small blood sample (approx. 3–5 ml serum). No fasting required. Ideal for children with autism who may not tolerate clinic visits.
The sample is packed in a temperature-stable kit and dispatched to our lab in Chandigarh via certified courier. Transit time varies by city — typically 24–72 hours.
On receiving the sample, our pathology team processes the FRAT assay under the direct supervision of Dr. Alok Gupta — MD Pathology (CMC Ludhiana), Senior Residency at PGIMER Chandigarh, with 12+ years of experience. Former consultant at Apollo Hospitals & Vimta Labs. Reports accepted at PGI, AIIMS, GMCH, Fortis & Max.
Your detailed FRAT Test report is delivered within 48 hours of sample receipt — via email and WhatsApp in PDF format. Delivered wherever you are in India.
Share the report with your neurologist, developmental paediatrician, or psychiatrist to plan the next steps — including folinic acid (Leucovorin) therapy if indicated.

The FRAT test measures two distinct types of autoantibodies. A positive result on either or both suggests brain folate transport disruption.
| Test Component | What It Detects | Clinical Significance |
|---|---|---|
| Blocking Folate Receptor Autoantibody | Autoantibodies that directly block folate binding to the FRα receptor | Positive result indicates folate cannot enter the brain via the primary receptor pathway |
| Binding Folate Receptor Autoantibody | Autoantibodies that alter receptor position, reducing folate uptake indirectly | Positive result confirms immune-mediated folate transport disruption |
A positive result on either or both components suggests your child's brain may not be receiving adequate folate, even if blood folate is normal. Your doctor may then recommend a trial of folinic acid (Leucovorin) therapy, which uses an alternate folate form that bypasses the autoantibody blockade.
Many parents are told folate levels are normal — but that doesn't rule out brain folate deficiency. Here's why the FRAT test is different.
| Parameter | Standard Serum Folate Test | FRAT Test (Folate Receptor Autoantibody) |
|---|---|---|
| What it measures | Amount of folate in the blood | Autoantibodies blocking folate transport to the brain |
| Can detect brain folate deficiency? | ✗ Blood folate may be normal even when brain is deficient | ✓ Specifically identifies the transport blockade |
| Relevant for Autism (ASD)? | ✗ Not specifically | ✓ Up to 75% of ASD children may test positive |
| Guides treatment? | ✗ No | ✓ Positive result supports folinic acid (Leucovorin) therapy |
| Fasting required? | Sometimes | ✓ No fasting required |
| Sample required | Blood (serum) | Blood (serum) — small 3–5 ml sample |
