Leaky gut in autism: when doctors may suggest the serum zonulin test
Gut problems are more common in autistic children than in other children, and researchers have long studied whether the gut lining is more "leaky" in some of them. When gut problems go on for months, doctors sometimes look more closely at gut health.
Doctors may suggest it when an autistic child has
- Long-standing constipation or diarrhoea
- Frequent tummy pain, bloating or gas
- Reflux or frequent vomiting
- A very limited diet, or reactions to certain foods
- Sleep or behaviour changes that seem to come with tummy discomfort
Why gut problems can be missed
Many autistic children cannot easily say they are in pain. Doctors note that tummy discomfort can show up instead as poor sleep, irritability or changes in behaviour. That is why some doctors look at gut health when these changes appear.

These signs have many possible causes, and most are not related to zonulin. Please speak to your child's doctor before booking. Not sure? Call us and we will explain what the test covers.
Leaky gut in autism: what the research shows

The gut lining is a wall of cells held together by tight seals. Zonulin is one of the signals that can loosen these seals. When they loosen, the lining lets more through than usual, which is what people mean by "leaky gut".
Researchers have studied the gut lining in autistic children for more than 15 years. Some studies found higher serum zonulin in autistic children than in other children, and higher levels in children with more severe symptoms. Other studies, including one published in October 2026, found no clear link. The studies so far are small, and scientists are still discussing exactly what zonulin tests measure. So this test can give your child's doctor one more piece of information, but it cannot confirm leaky gut or explain autism on its own.
Read the research in detail
How zonulin affects the gut lining
Laboratory and animal studies describe how zonulin loosens the seals between gut cells:
- Zonulin is released. Cells of the gut lining release zonulin, the precursor of a blood protein called haptoglobin 2.
- It switches on receptors. Zonulin activates a receptor called PAR2 on the gut cell, which switches on a second receptor, EGFR.
- The cell skeleton shifts. Signals inside the cell reorganise its internal frame of actin filaments.
- The seals pull apart. The tight-junction proteins that hold neighbouring cells together (ZO-1 and occludin) separate.
- A small gap opens. More can pass between the cells. In animal studies this effect was reversible.
These steps come mainly from cell and animal research. They explain why zonulin is studied as a marker, but a blood level alone cannot show how much this is happening in a child's gut.
Studies that found higher zonulin
A 2017 study in Turkey of 65 children found higher serum zonulin in autistic children than in other children, with higher levels in more severe autism. A 2025 study in Egypt found a similar pattern, including a link with gut symptoms. A 2020 review that pooled five studies of children with autism, ADHD and OCD found higher levels overall.
Studies that found no difference
A 2017 study in Poland found no difference in serum zonulin between autistic and other children. A 2014 UK study of over 130 children found no difference in gut permeability measured with a sugar-drink test. A study published in October 2026 found that zonulin levels did not match how severe a child's gut symptoms were, and its authors describe serum zonulin in autism as still debated.
The question about the test itself
In 2018 and 2019, two independent research groups found that widely used zonulin tests pick up a family of closely related proteins, not only zonulin itself. Kit makers now describe these tests as measuring "zonulin family peptides". This is one reason doctors read the result together with symptoms and other tests.
What this means for your child
A zonulin result is one clue, not a diagnosis. It is most useful as part of a wider look at gut health, guided by your child's paediatrician, paediatric gastroenterologist or treating doctor.
High zonulin levels in coeliac disease, IBS, IBD and diabetes: what research shows
Zonulin is not studied only in autism. Over the past 20 years, researchers have measured it in several gut, metabolic and immune conditions where the gut lining may be affected. Doctors sometimes request the test while looking into these conditions, alongside the standard tests for each.
Gut
- Coeliac (celiac) diseaseZonulin research began here. Laboratory studies found that gluten (gliadin) can trigger zonulin release from gut cells.
- Non-coeliac gluten sensitivityA 2020 multicentre study found higher zonulin in people with gluten sensitivity than in healthy people and in people with IBS.
- IBSResults are mixed. Another 2020 study found that serum zonulin could not reliably identify IBS or gluten sensitivity.
- IBD: Crohn's disease and ulcerative colitisA 2019 pilot study found higher serum zonulin in both Crohn's disease and ulcerative colitis than in healthy people.
Metabolism
- Type 1 diabetesIn a 2006 study, 42% of people with type 1 diabetes had raised zonulin, and in some people the rise came before diagnosis.
- Type 2 diabetesHigher levels were found in people newly diagnosed with type 2 diabetes (2014).
- Obesity and insulin resistanceZonulin rose with body weight and with lower insulin sensitivity in a 2012 study.
- Fatty liver disease (NAFLD)Higher levels were found in children with fatty liver, rising with the amount of liver fat (2014).
Immune & brain
- Rheumatoid arthritisA 2020 study found higher zonulin in people with rheumatoid arthritis, and in some people before joint swelling began.
- Autism and ADHDSee the autism research above. A 2020 review also reported higher levels in children with ADHD.
In each of these conditions, zonulin is a research marker. It does not diagnose any of them, and each has its own standard tests, such as the coeliac blood test (tTG-IgA), HbA1c for diabetes and stool calprotectin for IBD. Your doctor decides which tests you need.

Serum zonulin test questions, answered
What is the serum zonulin test?
It is a blood test that measures zonulin, a protein linked to how tightly the cells of the gut lining are sealed together. Your doctor reads the result with your child's symptoms and other tests. At Altus Lab it costs ₹9,500 with free home sample collection.
Is the zonulin test a leaky gut test?
It is often called a leaky gut blood test, because zonulin is linked to the seals in the gut lining. But no single test can confirm "leaky gut", and leaky gut syndrome is not a recognised medical diagnosis. The result adds information for your doctor.
Is the zonulin test a test for autism?
No. It does not diagnose autism or tell whether a child is autistic. Doctors may suggest it when an autistic child has ongoing gut problems, to look more closely at gut health.
Which other conditions is the zonulin test used for?
Besides autism, researchers have studied serum zonulin in coeliac disease, non-coeliac gluten sensitivity, IBS, inflammatory bowel disease (Crohn's disease and ulcerative colitis), type 1 and type 2 diabetes, obesity, fatty liver disease and rheumatoid arthritis. Higher levels have been reported in some of these groups, but results vary between studies. Zonulin does not diagnose any of these conditions; doctors read it alongside the standard tests for each.
Why would an autistic child need a gut test?
Gut problems such as constipation, diarrhoea and tummy pain are more common in autistic children, and some children cannot easily say they are in discomfort. Doctors may use gut tests to look into these problems.
What is leaky gut in autism?
"Leaky gut" is the everyday name for increased gut permeability, when the gut lining lets more through than usual. Some studies have found signs of this in a group of autistic children, especially those with gut problems; other studies have not. Researchers are still working out how often it happens and what it means. Serum zonulin is one of the markers studied.
What is the zonulin test price in India?
At Altus Lab, the serum zonulin test costs ₹9,500, with free home sample collection included. The price is the same in every city in India, with no hidden charges.
Is the zonulin test available in India? Can I get it in my city?
Yes. We collect samples at home across India, including Bengaluru, Hyderabad, Chennai, Kochi, Delhi NCR, Mumbai and Pune, and test them at Altus Reference Lab in Chandigarh. Call +91 628 421 4977 to confirm your area.
Does my child need to fast before the test?
No fasting is needed. Our team calls before the visit to confirm the details and explain what to expect.
How is the sample sent to the lab?
Zonulin is not stable at room temperature, so the serum is separated and kept frozen at −20 °C, then air-lifted to our reference lab in Chandigarh through cold-chain logistics.
How long does the report take?
About 2 weeks from the day the sample reaches our lab. We send it on WhatsApp and email.
Is the zonulin test reliable?
Every report is supervised by an MD pathologist at our NABL-accredited lab. Research groups have shown that zonulin tests pick up a family of related proteins, not only zonulin, so the result should be read with your child's symptoms and other tests.
What is a normal zonulin level?
Normal ranges depend on the test kit and the lab. Your report shows our lab's reference range next to your child's result.
What does a high zonulin result mean?
A high result means the level is above our reference range. It has been linked in research to changes in the gut lining, but it does not confirm a diagnosis. Please show the report to your child's doctor.
What if the result is normal?
A normal result does not rule out gut problems. Your doctor may suggest other tests depending on your child's symptoms.
Which doctor should we see after the report?
Your child's paediatrician, a paediatric gastroenterologist, or the doctor who advised the test.
Should we start a gluten-free or casein-free diet if zonulin is high?
Please don't change your child's diet based on this result alone. Studies of these diets in autism show mixed results, and restricted diets need medical and dietitian guidance to keep a child well nourished.
Blood or stool zonulin: what is the difference?
Zonulin can be measured in blood (serum) or stool. Altus offers the blood test, collected at home. Stool zonulin has been studied less than the blood test.
Can the zonulin test be done with FRAT or OAT?
Yes, if your doctor advises them. They are separate tests, booked separately, and the samples can be collected in one home visit.
Will my doctor accept a report from Altus Lab?
Altus Lab is NABL accredited, and our reports are accepted at PGI, AIIMS, GMCH and major hospitals. Every report is supervised by Dr. Alok Gupta, MD Pathology, Ex-PGIMER, Chandigarh.
For doctors: sample handling and interpretation
- Analyte: zonulin family peptides (ZFP) in serum
- Processed at: Altus Reference Lab, Chandigarh
- Reference range: reported with each result
- Sample handling: serum separated promptly, frozen at −20 °C, air-lifted frozen
- Interpretation: independent studies (Scheffler 2018; Ajamian 2019) found that widely used commercial zonulin tests detect zonulin-family proteins rather than pre-haptoglobin 2 alone. Results should be read with clinical findings and other investigations.
